For Microsoft Employees:

Jumpstart provides eligible Microsoft employees, spouses or same sex domestic partners who are enrolled in Premera Blue Cross insurance, a comprehensive 6 month weight loss program. The program involves a food-based diet that we will outline, safe and effective optional FDA-approved appetite suppressant medication, visits with bariatric (weight specialist) psychologists and dieticians, and a series of sessions with excellent personal trainers. 80% of the $7,200 cost of the 6-month program is paid for by Microsoft. The beneficiary is responsible to pay the remaining $240 per month or $1440 over the course of the 6-month program.

Eligibility requirements for participation include:

• Obese defined as a body mass index (BMI) > 30 or
• Overweight with a BMI of 27 – 30 but with two or more of the following co-morbid conditions:
  • Diabetes
  • Hyperlipidemia
  • Hypertension
  • Coronary artery disease
  • Congestive heart failure
  • Depression

Interested Individuals:

Interested individuals must first schedule an appointment with your primary care physician to pre-certify for eligibility through confirmation of your BMI and co-morbid conditions. After that appointment, your physician will need to fax the completed Premera Weight Management Recommendation Form 011994 to Premera at (800) 866 4198. Once Jumpstart has received confirmation of your eligibility through Premera, simply call us to schedule your initial visit with our team.
